The questions we hear most
What services do you provide?
We offer Minnesota HCBS in three groups: Basic Support (24-hour emergency assistance, adult companion, homemaker, night supervision, respite, individual community living support, individualized home supports), In-Home Support (semi-independent living skills and individualized home supports with optional training and family training), and Employment Services (exploration, development, and support).
Who qualifies for services?
Adults eligible for Minnesota Home and Community-Based Services waivers — including those who qualify through their county case manager. If you're unsure, call us and we'll help you figure it out.
Do you accept county referrals?
Yes. We work closely with case managers across Minnesota counties. Use the referral form or call us directly.
Can family members make referrals?
Absolutely. Family members and self-referrals are welcome. We'll coordinate with your case manager from there.
Are services personalized?
Every plan is built around the individual — their goals, routines, preferences, and the people in their life. We don't deliver one-size-fits-all care.
